The following does not apply to military and test pilots.

1. OVERVIEW

• Each civilian pilot participating in the Haute Voltige® Events must hold a Haute Voltige® Pilot Display Authorisation (which is necessary to obtain an FAI World Grand Prix Licence).

• The Haute Voltige® Pilot Display Authorisation will be delivered after submission of the current Haute Voltige® Entry Application to the Haute Voltige® Head Office. It will be subject to the agreement of the Haute Voltige® Director who will propose a specific Agreement to each pilot, detailing the flight programme and eventual Sponsorship conditions.

• The Entry Application and its Pilot Display Authorisation, the Haute Voltige® Operating Regulations, the Authorisation to Publish and the Pilot’s Agreement will represent the Terms and Conditions applicable to an Haute Voltige Event, in addition to the FAI World Grand Prix Sporting Code when applicable.
However, please note these documents can be superseded by local law/regulations. If so, all amendments will be detailed in a briefing prior to the Event and acknowledged by each pilot through his/her signature of the Event/Airshow Waiver.

• All personnel assisting the applicant on site, including his/her Technician, will have to be listed in the application and will be placed under the legal responsibility of the applicant, after having been agreed by the Haute Voltige® Director and the Promoter.

• The applicant must make sure that his/her Technician and personal assistants are covered by 3rd party liability and personal Health and Injury insurances, including Repatriation. He will be responsible for paying all expenses incurred by his Technician and Assistants.

• The list of Assistants can be amended until 10 (ten) days before the first day of the Event.

• The Haute Voltige® Head Office must receive all amendments to this application in writing.

Validity
The Haute Voltige® Pilot Display Authorisation is valid for one event and must be renewed for each event.

Entry fee and Payment
The Haute Voltige® Entry fee for this application is set up at USD 1'000 per Pilot & Technician and USD 1'000 per Assistant.
When not sponsored by Haute Voltige®, all payments due to Haute Voltige® are to be made via bank transfer upon receipt of an invoice from the Haute Voltige® Head Office which will indicate the amount and the terms of payment.

7. APPLICATION for the HAUTE VOLTIGE EVENT

      7.1.       I, the undersigned, hereby apply for entering the Haute Voltige® Event with all Technician and Assistants listed above and to be placed under my responsibility;

      7.2.       I, the undersigned, hereby agree on behalf of myself and everyone associated with my participation in the Haute Voltige® Events to observe all the regulations in force at the Event and a true spirit of sportsmanship.

      7.3.       I, the undersigned, hereby apply for a Haute Voltige® Pilot Display Authorisation for the Event and an FAI World Grand Prix Licence;

      7.4.       I, the undersigned, hereby confirm that I have read and understood the Haute Voltige® Operating Regulations, and that I will comply with their obligations;

      7.6.       I, the undersigned, hereby certify the information on this form is true and accurate and I understand that the above information, if misrepresented or incomplete, may be grounds for termination of my Haute Voltige Pilot Display Authorisation and penalties.

      7.7.       I, the undersigned, hereby certify that I will inform Haute Voltige® in writing, of any change in my medical status occurring in the period between the day my medical certificate was delivered and the last day of the Event. I understand and agree that the Haute Voltige® Director has the right to withdraw my Haute Voltige® Pilot Display Authorisation if I cannot provide a new medical certificate valid until the end of the Event. Should I neglect to inform Haute Voltige® in writing, then I will not be entitled to make any claim against Haute Voltige®, FAI, or the Event organisers and accept to undertake all consequences of this negligence, including reduction or cancellation or sponsorship monies.
